This authorization is required anytime a deceased person has been sent to the Dallas County Medical Examiner’s Office (Institute of Forensic Sciences).

The Dallas County Medical Examiner will not release a body until they have a signed release in their possession. Just complete the fields below and our software will fill out the release for you.
